COVID Checkup

It's time for another COVID checkup. I say this for two reasons, one personal and one media-related. The personal is that I just got my second booster shot this past weekend. Which I would have done anyway, because I personally trust medical science and want to be as protected as possible against the pandemic; but in the past few days I might have been more motivated to do so, if all the information I got was from television news. Because, according to them, it's time for everyone to panic once again.

The news media has taken a lot of flak for their pandemic reporting, some of it justified and some not. This is a deadly disease which has swept through the American population in multiple waves and is close to having caused one million American deaths. That's as serious as it gets, obviously. The number of deaths is almost certainly a lot higher than that, but when the official number does push through the seven-digit threshold, it will doubtlessly be big news. It is natural to note when such a milestone number has been reached, but what is less natural is to treat all COVID news as bad -- or to exclusively report on the bad at the expense of any good news at all. Which sums up the viewpoint of television news for roughly the past year or so.

This is perhaps most notable right now. At first, most of the COVID news was bad, some of it horrifyingly so. There were no vaccines or accepted treatments, and the medical profession was essentially in the dark on what to do. The hospitalization rates and death rates were correspondingly high, which was tragic to see. The last wave of this situation came in the winter of 2020-2021. Treatments were slowly appearing, and the vaccine had finally been approved, but both were mostly too late for that first winter's surge. Six months later, we all experienced what appeared to be COVID's final retreat, which culminated in July as we all returned to Independence Day picnics and threw off our masks.

Since then, the situation has changed. More and more people got vaccinated. More and more people had built up at least some degree of natural immunity by already having survived a bout of COVID. But Delta swept back through the population anyway, leading to the reinstatement of masks and other pandemic restrictions, right after everyone thought they were gone forever. Delta was a lot more deadly than the initial strain, and it was indeed a reason for both public attention and concern. This rollercoaster then continued, as just when it appeared Delta was finally waning, Omicron appeared. The Omicron mutation was serious news, since it was a lot more contagious than the original strain or Delta, although (thankfully) it was also less lethal than Delta. But the caseload shot through the roof in a way not seen in any previous wave, so it gave the news media plenty of breathless reporting opportunities.

New cases in January of 2021 hit a daily average of around 250,000. During Delta, this number never rose above around 160,000. But in the Omicron wave it astonishingly topped 800,000 new cases per day. That's news, obviously, and it was indeed a cause for concern.

But what was interesting at the end of the Omicron wave was how quickly the story disappeared from television when the news turned good again. For a few weeks, the cameras sought out the increasingly-hard-to-find overcrowded hospitals so they could run stories about how "the wave isn't over yet!" but when the wave really did recede everywhere, no stories at all appeared. Good news isn't newsworthy to the news media, in other words.

But now they think they've got another spike to report on, so they've gone back into overdrive. The stories this week have all led with: "Cases rising!" They even hint that the problem could be much worse -- because "most cases aren't even reported anymore, due to all the at-home test kits!" Philadelphia was the big story, since they announced a return to a mask mandate beginning next week.

Well, yes, all of that is true, but could we please keep things in just a wee bit of perspective? Is that too much to ask? Because while we may indeed be at an inflection point on the graphs, this may not in fact be all that big a deal at all. It's simply too early to tell -- a nuance you will find conspicuously absent from most of the reporting.

Let's take a look at the data, from the New York Times COVID tracking page (which is free for all to access, as it's not behind their paywall). Looking at the big chart of the rolling average of daily new reported cases, we can see that the numbers have risen over the past ten days. On April 3rd, we hit the lowest daily number since last summer, with only 26,992 cases (on average) reported daily. Yesterday, this number was up to 32,218 (although it is down again today to only 31,567). But please note the rest of that graph -- the missing context. Again, at the height of Omicron, we were seeing over 805,000 cases per day.

The media is seemingly a big fan of the maxim about three types of lie ("a lie, a damned lie, and statistics") whenever they toss out scientific numbers, since (again) bad news is what they like to focus on. So (once again) they are spotlighting the worst area of the country (currently New England), where they breathlessly report the case rate is "up over 50 percent in recent days!" That certainly sounds a lot scarier than stating "the national case rate has inched up from 3.3 percent of the high reached during Omicron to 4.0 percent," which is also the truth according to the data.

Even when you look at the individual state data, you can clearly see that while the case rate has been rising in certain states, we're still not at the point where alarm bells should loudly be ringing. Take Rhode Island, which is currently the worst state in the graphs the New York Times uses to measure state-by-state impacts. The measurement used in these charts is "percentage of the peak reached in the winter of 2020-2021," which is somewhat strange, but the chart would look almost the same no matter what scale you used to draw it. Rhode Island's case numbers have risen from a low of only 18 percent of the first winter peak to 42 percent. That is more than doubling, obviously. Rhode Island is an interesting state to use as an example, since it saw the biggest spike under Omicron of any state -- when it hit a whopping 669 percent of the first winter's peak. A change of even 24 percent doesn't look so bad, when the recent past is taken into account. And for further context, in most states -- and in the nation as a whole -- the graph line is still either flat or continuing to fall. Using a different measurement, no state in the country is now above 50 new daily cases per 100,000 people -- which is a lot lower than where it was during Omicron (when it spiked to many hundreds of cases per 100,000 per day). Only three states and the District of Columbia are even above 30 cases per 100,000 -- every other state is below this mark. Most states (32 of them) are in fact in the single digits in this metric.

Basically, it is way too soon to tell whether the BA.2 variant of Omicron is going to be a huge problem or not. Right now, "probably not" is likely the safest bet. Of course, this could change quickly if the numbers spike a lot more dramatically than they are spiking now (which did indeed happen with Omicron), but even then things might not be too bad.

I say this because even though the case numbers are rising in New England, the number of people hospitalized is not spiking. Neither is the death rate -- but that's immaterial at this early stage, since deaths always lag behind infections and hospitalizations by weeks (meaning such a spike wouldn't even show up for a few more weeks, at the earliest). This could be due to the BA.2 variant being no more dangerous than the original Omicron mutation, but at least part of it is where we are as a population right now.

A full two-thirds of the American population (66 percent) is now fully vaccinated (which means the initial two doses, without taking boosters into account). At least 77 percent have gotten at least one dose. While these numbers aren't as high as some developed nations, even this is a pessimistic way to look at it. One segment of the population -- children under the age of six -- has not even been approved for vaccinations. When you count only the population that has been approved, the numbers improve to 70 percent fully vaccinated and 82 percent with at least one dose. Added to this metric is the fact that there have been over 80 million American cases reported during the entire span of the pandemic (although some people have contracted it multiple times, so that doesn't equate to the number of people who have had COVID). A big chunk of those cases happened during that gigantic Omicron spike -- quite recently, in other words. People who have had the virus and recovered are at least partially protected by the body's own natural immunity.

To put all this another way, America may not be fully there (experts disagree about where to draw the line) but we are about as close to "herd immunity" as we are going to get. This means that even though the case numbers may go up in a post-Omicron spike (which hasn't really happened yet -- it's hard to call a rather gentle upturn a "spike"), the number of hospitalizations and deaths likely won't (or at least not too seriously). People will get a little sick, stay at home for a while, and then get better. The hope is that eventually this becomes the norm, and COVID fades into the generic background of being just "a seasonal flu" that doesn't cause mass deaths (the way it first did).

But it's hard to keep this context -- that the recent increases should cause a little concern, perhaps, but certainly not some sort of wild panic -- when hearing about the numbers on the evening news. The biggest lack of context is the fact that it is still far too early to say, one way or another, how impactful the BA.2 wave is even going to be. Will it truly begin spreading wildly, causing a true spike in the numbers? Or will it just be a blip upwards for a month or so that eventually comes right back down again? Nobody knows, at this point. Because it is still too early to tell.

As I said, I write all of this after getting my own second booster shot (which I would have done no matter what the numbers were doing) and then seeing the news reporting about the pandemic over the past few days. It all left me yearning for the days when there were actual "science reporters" -- journalists with enough mathematical and scientific training to know how to read and interpret a chart of data, in other words -- who would get assigned these stories rather than the news anchors and morning-fluff-news "reporters" who are now covering it. Because if you take the time and look at a few graphs, it's pretty easy to see that there really is no reason for any alarm, at least not yet. Which is a fact notable in its absence from the actual news reports.

    There is evidence that the immunity conferred by a first booster starts to wane after about 4-5 months, which is why the recommendation of a 2nd after 6 months. Or possibly this variant evades it somewhat. It seems to have a much shorter exposure time, sometimes just a couple of days. People who had Covid the first time, or one of the other variants, are going down with this variant as well.

    Case in point, my OH (pushing 80) contracted this variant a couple of weeks ago, about 5 months after booster 1, along with a lot of other people who had escaped the original covid and first variants. He almost certainly picked it up in a choir rehearsal where everyone was singing unmasked. The good news is that it was no worse than a bad cold and only lasted a few days for him, as for most, but that doesn't mean it isn't dangerous for some. I seem to have escaped this time--I still have about a month to go before booster 2.

    (For what it's worth, we both had Covid fairly early on, although because we had the 'wrong' symptoms, no fever in particular, we weren't tested at the time. My OH picked it up at choir or at the last concert before lockdown that time, too; masks weren't generally being worn then. Fortunately, we were not ill enough to need medical care, just go-to-bed ill for a few days. We got confirmation that it was Covid when we took part in a mass test a bit later on which showed we both had disease antibodies.)
